Agreement to Terms
By accessing and using the imaging services provided by University Diagnostic Institute (UDI) through USCAN, the patient acknowledges and agrees to the terms and conditions outlined below. These terms govern the patient’s use of USCAN’s services and their participation in imaging procedures.
The patient acknowledges that he or she has not been provided with a referral for imaging from an external healthcare provider. The decision to undergo imaging at University Diagnostic Institute is based on the patient’s personal healthcare choices.
Before receiving imaging, the patient understands that:
An initial consultation will be conducted at University Diagnostic Institute to determine whether imaging is appropriate for the patient.
Imaging will only be provided if a UDI healthcare provider determines that it is appropriate based on the patient’s medical history and needs.

Patients receiving imaging will be provided with a copy of their results upon completion of the MRI scan.
An MRI alone does not diagnose all medical conditions and should not be solely relied upon for medical decision-making.
MRI interpretations may vary between radiologists, and results could differ depending on the professional reading the scan.
If deemed necessary, it is recommended that the patient obtain a second opinion regarding the MRI results for further evaluation.
University Diagnostic Institute will not provide treatment or follow-up examinations after imaging.
It is the patient’s responsibility to seek consultation from an appropriate healthcare provider to review MRI results and discuss potential treatment options.
Payment and Pricing
- Patients acknowledge that payment is required before the time of service.
- USCAN offers transparent, self-pay pricing, and there are no hidden fees.
- Payment options include credit/debit cards, HSA/FSA cards, or other accepted forms of payment.
- USCAN does not accept health insurance for imaging services. All patients are responsible for full payment at the time of service.
Privacy and Data Protection
- USCAN follows HIPAA compliance to protect patient confidentiality.
- Patient information and imaging results will be securely stored and only shared with the patient or authorized parties.
- USCAN will not sell or disclose patient data to third parties without consent, except as required by law.
Limitations and Liability
- USCAN and University Diagnostic Institute are not responsible for medical diagnoses—imaging is provided solely as a diagnostic tool.
- USCAN is not liable for treatment outcomes based on MRI results or for any misinterpretations by external providers.
- Patients accept responsibility for seeking appropriate medical follow-up and further evaluations if needed.
